THE USE OF THE 4097 FORM

Do you know about the 4097 form? We would start this new post reminding you how you could obtain the medical evaluation for international travelling through this simple survey. This useful questionnaire is an easy way that can help you prevent health problems before, during and after your travels. The 4097 form is very helpful to maintain travellers’ and foreign assignments’ health and safety’. You must be aware that minor health problems can produce important inconveniences during or after travel periods if they are not properly addressed previously. So, in order to avoid future problems and have a good follow-up of your health, please fill the form before starting your travel.

The medical evaluation contained in the form is designed to estimate and promote a person's fitness for international travel or foreign assignment through assessment of current health status; identify existing medical conditions which may require treatment or investigation prior to travel; and provide travel advice regarding immunizations, medications and travel issues or concerns
Complying with the 4097 form you will obtain Medical Clearance for International Travel and Foreign Assignment.
It is the traveller’s responsibility to ensure that this medical clearance is obtained at least every 3 years.
Medical records will be kept in the exclusive custody and control of AP medical staff or one’s own personal practitioner.

The 4097 form consists of three pages:

The second and third pages include 3 sections (travel history, family doctor, and personal health history) and should be completed by the traveller and reviewed by the medical practitioner. The first page should be completed by the same medical practitioner and indicate possible advice and recommendations.
You will have to fill out only the second and third page. This requires only 10 minutes of your time.
Then, please notify your Occupational Health Doctor, General Practitioner or the local person responsible for this area, depending on your current location.
